Patient with valid OHIP coverage
If you like to be referred to IVF Canada for your fertility care, you should obtain a referral request from your family physician, or obstetrician/gynaecologist, or other specialist. You may download the form of REQUEST FOR CONSULTATION that the referring doctor would fill out your medical information and send to us. Click Here for a referral request letter.
- Referrals can be faxed to 416-321-1239 or emailed to info@ivfcanada.com.
- If you encounter any problem obtaining the referral, please contact Vivian Li ( IVF Canada marketing manager) via email: vivian@ivfcanada.com for assistance.
- Upon receiving your referral, we will contact you within 48 hours to book your initial consultation, an appointment will usually be scheduled within 2 weeks. Please note: if you do not receive the appointment notice in 1 week, please call our office at 416-754-1010 in case your request for an appointment was not received.
Patients without OHIP coverage
A referral letter is NOT required. Please request an appointment through one of the following methods:
- In person or
- By telephone at 416-754-1010 or
- By filling in the registration form, please Click Here or
- By sending a request through the email address info@ivfcanada.com.
- Upon receiving your request, we will contact you within 48 hours to book your initial consultation, an appointment will usually be scheduled within 2 weeks.
